The most effective and correctly constructed treatment option is complex pathogenetic therapy, in which the effect of one treatment method is complemented and enhanced by the effect of others.
It should be noted that physiotherapy is not an independent treatment option for endometriosis, but when used according to indications, it is an effective additional method that allows you to enhance the effect of drug therapy, increase the interval between courses of hormonal therapy and thereby reduce the severity of its side effects, reduce the incidence of postoperative complications, improve the patient’s condition, solve reproductive problems.
In Russia, natural and preformed physical factors have long been used for the treatment of endometriosis, which is due to the presence in our country of unique natural sources of radon and proven methods for its medical use, as well as the existence of an established school of hardware physiotherapy. Our search for literature on the use of any physical factors for the treatment of endometriosis, published in the last 10 years, showed that in the world physiotherapy is practically not used for the treatment of gynecological diseases, in particular endometriosis, although in neurology, cardiology, and traumatology physical factors are used very widely and effective for the rehabilitation of patients. At the moment, there are no results of randomized controlled studies on the use of physical factors in other areas of medicine, which is due to significant difficulties that arise during their implementation associated with the use of physiotherapy (different equipment, the use of different techniques, the impossibility of unifying natural factors, the difficulty of isolating the effect of physiotherapy in evaluation of the results of complex treatment) and the high cost of such studies. Nevertheless, Russia has accumulated extensive clinical experience in the effective use of natural and preformed physical factors in the treatment of endometriosis, which allowed us to provide this material for use in clinical practice.
Preformed and natural physical factors used to treat patients with endometriosis:1. Low frequency pulse currents - diadynamic, sinusoidal modulated, fluctuating, interference, rectangular (electrotranquilizer), transcutaneous electrical neurostimulation. Pulsed currents have a pronounced analgesic and sedative effect, but do not provide an estrogen-stimulating effect. Electrophoresis of medicinal substances, in particular iodine, using pulsed currents allows the administration of small doses of the drug, and in small doses promotes its deposition in the skin for up to 3 weeks. and its gradual entry into the blood, its accumulation in the pelvic area, is of great importance in the chronic course of the disease, since, accumulating in the source of inflammation, iodine ions inhibit alteration and exudation, stimulate the processes of reparative regeneration (structure collagen and elastic fibers of connective tissue). Under the influence of iodine, the balance of releasing hormones of the pituitary gland and sex hormones is normalized. However, pulsed currents cannot be used if the patient has urolithiasis or cholelithiasis. Treatment begins on days 5–7 of the menstrual cycle (to avoid an inadequate reaction in the form of acyclic bleeding, increased pain) against the background of mechanical contraception with a condom or COC. The aftereffect period during treatment with pulsed currents is at least 2–4 months.
2. Low-frequency magnetic and electromagnetic fields with different characteristics - magnetotherapy (constant, alternating, pulsating, pulsed, traveling magnetic field), pulsed electromagnetic field. Magnetotherapy, in the absence of thermal and estrogen-stimulating effects, has a pronounced local anti-inflammatory, hypocoagulating, desensitizing, analgesic, vasoactive, metabolism-stimulating effect due to the restoration of the permeability of the membranes of inflamed tissue cells, the elimination of edema and the pain caused by it. The high magnetic permeability of biological tissues ensures the penetration of magnetic fields to almost any depth, which is very important for large volumes of surgical intervention (when using a magnetic field with a frequency of 50 Hz and an inductance of 35 mT). Both local (in the pelvic area) and general magnetic therapy are used, which provide a general sedative effect. The pulsed electromagnetic field normalizes the state of the central and autonomic nervous system and improves microcirculation. Magnetic therapy is carried out mainly in the early postoperative period.
3. Electromagnetic oscillations of the optical (light) range - short-wave ultraviolet radiation (AUV radiation), laser radiation (with a wavelength of 620–1300 nm). Local exposure to EF radiation on the area of the postoperative wound gives a pronounced bactericidal effect, stimulates the phagocytic activity of leukocytes, causes a short-term spasm of the capillaries, and then dilation of the subcapillary veins, and accelerates the process of epithelization of the wound. The direct short-term effect on the wound of laser radiation has a bactericidal, anti-inflammatory, analgesic, and pronounced biostimulating effect on reparative processes due to the normalization of microcirculation, reduction of tissue edema, and activation of metabolic processes in the source of inflammation. With a combined effect - magnetic laser therapy - the physiological and therapeutic effects are summed up and potentiated, and the depth of penetration of laser radiation increases. It is most advisable to use these factors in the early postoperative period.
4. Balneotherapy – general radon and iodine-bromine baths. To treat endometriosis, water with an average radon concentration of 40 to 200 nCi/l (1.5–7.5 kBq/l) is used, often in combination with microenemas and vaginal irrigation with radon water (for retrocervical endometriosis). Indifferent baths are used - water at a temperature of 33–36°C. The main clinical effect of radon is its effect on the nervous system at all levels of regulation - inhibition, sedative and pronounced analgesic effects. Radon normalizes the ratio of sex steroid hormones and gonadotropins, reduces the level of hyperestrogenism, and increases the content of progesterone. It gives a pronounced anti-adhesion effect, normalizes central and regional hemodynamics in the pelvic area. This is the most powerful and effective factor influencing endometriosis with a long aftereffect period of at least 6 months, which allows you to increase the intervals between courses of hormone therapy. Iodine-bromine waters (iodine concentration of at least 10 mg/dm3 and bromine concentration of at least 25 mg/dm3) are also used in the form of baths and vaginal irrigations in indifferent thermal conditions (33–36°C). They have a pronounced sedative and anti-inflammatory effect, affect the function of the thyroid gland, lower blood pressure, and suppress hyperestrogenism. Iodine-bromine waters help to increase the threshold of pain and tactile sensitivity, causing a long-term decrease in the severity of pain. The aftereffect period of such baths is up to 4 months.
5. Hydrotherapy – pine, bischofite baths, the action of which is based on thermal, mechanical and chemical irritation of skin receptors and reflex vascular changes. Therapeutic baths provide a sedative, analgesic, and antispastic effect, which are enhanced by the addition of various substances (pine extract, bischofite) to fresh water. For endometriosis, indifferent baths are used (water temperature 33–36°C), which have the least effect on heat balance, thermoregulation and metabolism. The aftereffect period of hydrotherapy is at least 3–4 months.
6. Climatotherapy. It should be noted that natural excessive insolation has a stimulating effect on the development and progression of endometriosis, which patients should be warned about, taking into account modern changes in weather conditions and the desire of patients to relax in climatic zones that are unusual for them. The use of physical factors that cause hyperestrogenism in the treatment of endometriosis is absolutely contraindicated: therapeutic mud, heated sand, paraffin, hydrogen sulfide, sodium chloride, turpentine, sulfide baths, estrogen-stimulating preformed physical factors (ultrasound, supratonal frequency currents, diathermy, indothermy, ultra- and ultra-high frequency), massage of the lumbosacral area of the spine, baths and saunas. Particular methods of conducting electrical procedures, balneo- and hydrotherapy are described in reference books.
Favorable features of the use of physical factors should also include: physiology, since physical factors are elements of the natural external environment, under the influence of which the body is constantly located; safety; lack of toxicity, side effects and allergic reactions; painless procedures; locality of action; individualization according to dosimetric parameters and methodological techniques; long aftereffect period; good compatibility with other medications, the possibility of using physical factors in therapeutic complexes of various types with potentiation of the effect of medications;
Economic efficiency.
The main indications for the use of physical factors in the treatment of endometriosis: Endometriosis stage I–II of spread, confirmed promptly; break during long-term hormone therapy. Physiotherapy in this case complements the main treatment option – hormone therapy. The goals of physiotherapy are to reduce the severity of pain, trophic, anti-inflammatory, anti-adhesive effects. The use of electrotherapy is indicated: therapy with low-frequency pulsed currents or iodine electrophoresis using pulsed currents. Procedures are carried out daily 1–3 times a day, a course of 20–30 procedures. It is advisable to combine electrotherapy with therapeutic baths (iodine-bromine, pine, bischofite). However, it must be taken into account that against the background of such therapy when using COCs, there is a risk of breakthrough bleeding. The risk of such an adverse reaction can be reduced by switching to exposure to electromagnetic and magnetic fields. Most often, physical therapy is carried out in between courses of hormonal therapy. Alternating courses of hormonal and isotherapy makes it possible not to use (or delay the start of taking) second-line hormones and to treat with gestagens or COCs.
Young age of patients. Physiotherapy in the treatment of adolescents can be used in several directions, in particular for dysmenorrhea as an empirical therapy. Physical factors do not produce side effects that are possible with the use of NSAIDs and hormones. With a confirmed diagnosis of endometriosis and hormonal therapy, physiotherapy can be used as an adjuvant option, which will reduce the hormonal load (and in some cases replace it) and the severity of negative consequences, increasing the intervals between courses of treatment, if possible, delay the timing of surgical intervention, and normalize the psycho-emotional state girls. Electrotherapy, general coniferous, bischofite baths are indicated. Iodine-bromine baths and radon therapy can only be performed when treating patients over 18 years of age with an established regular menstrual cycle.
Alternative treatment for intolerance to medicinal (hormonal) drugs and somatic burden, especially in older patients. Rarely, there are situations of either hormone intolerance due to various somatic pathologies, severity of side effects, or refusal for personal reasons to take hormones. In these cases, the use of physical factors may be an option in the treatment of the patient. Electropulse therapy or iodine electrophoresis using pulsed currents, which give a pronounced analgesic effect, is indicated. It is advisable to combine electrotherapy with therapeutic baths (iodine-bromine, pine, bischofite). Radon therapy may be an option for such patients.
Adjuvant therapy for chronic pelvic pain syndrome. Physiotherapy for CPP is always adjuvant, since it cannot provide an analgesic effect comparable to the effect of hormonal therapy. Considering the various pathogenic mechanisms of the formation of pain syndrome in endometriosis, the use of physical factors is possible only against the background of ongoing hormonal therapy, in between courses of hormones, and after operations. They use pulsed currents, iodine electrophoresis, hydro- and balneotherapy. This complex (2–3 procedures per day and a general therapeutic bath) provides a pronounced analgesic and general sedative effect with an aftereffect period of 2–4 months. There are no side effects observed with the use of hormones. Pain syndrome is the main indication for rhodonotherapy in the form of general baths and vaginal irrigation. The aftereffect period of radon therapy is at least 6 months.
Prevention of adhesions and possible inflammatory complications in the early postoperative period during surgical treatment of endometriosis. Physiotherapy prevents the delay and deepening of the first phase of the wound process - the inflammation phase, ensures an uncomplicated course of the postoperative period due to the end of the first phase of the wound process as early as possible physiologically, allows us to limit ourselves to standard drug therapy, and helps to reduce the length of the patient's stay in the hospital. It is advisable to start treatment already on the 1st day after surgery. Preformed physical factors are used (low-frequency magnetic therapy, laser therapy, magnetic laser therapy, EF radiation) in an intensive mode. In the absence of bloody discharge from the genital tract, it is advisable to use intracavitary energy conductors, the efficiency of which is higher than cutaneous ones.
In the long-term postoperative period in the complex treatment of adhesions in the pelvis, chronic pelvic pain, clinical manifestations of progressive endometriosis. Considering the multifactorial pathogenetic mechanisms of the formation of the clinical picture of the disease, the surgical treatment previously performed on the patient, hormonal therapy, and physiotherapy are used as adjuvant therapy. In this case, it is possible to carry out both outpatient electrotherapy in combination with the use of resort factors in non-resort conditions, and treatment in specialized sanatoriums, where preformed physical factors are used, natural resort factors (climatic, balneo- and hydrotherapy), and psycho- and kinesitherapy are performed. Adequate surgical intervention provides a positive clinical effect in 84.4% of cases, the pregnancy rate is 23.1%, and the disease relapse rate is 20.5%. After combination therapy using surgical, hormonal and spa treatment, clinical effect is achieved in 83.8% of cases, the pregnancy rate is 26.1%, and the disease relapse rate is 16.2%.
Contraindications to the use of physical factors for endometriosis: cystic forms of endometriosis - endometrioid ovarian cysts; all forms of endometriosis that require surgical treatment; Stages III–IY of the spread of endometriosis, since physiotherapy initially cannot be effective and should be abandoned;
Profound psycho-emotional disorders, neuroticism of the patient against the background of the underlying disease, corrected with psychotropic drugs.
Summary1. Laser therapy
